This is the quickest way I've found (withouth cheating) to finish the level. Upon entering the looney bin, talk to the orderly and get admitted. As soon as the doors open, run like a scalded dog up to the professor's office. Run straight in and grab the key. When he starts to jabber at you, hit the escape key to kill the custscene and buy a few seconds. Do the same thing when the SWAT scene strarts. Anyway, run into the small office directly across from his. Grab the shotgun and ammo, then wait.

Eventually a SWAT guy or two will mosey in. Blow one or both away and steal their clothes and weps. With that done, you have free run of the building. You can go load up on ordnance upstairs, or get on with talking to the inmates. Either way, a couple of options are to cap isolated SWAT guys and orderlies.

Instead of letting the prof get away, run through the second door ahead of you when the guard lets you in (instead of up the stairs). Here is an alternate set of stairs which lead to an office. Through a second door is another office whch contains a shotgun, uzi and syringe (works much like the fibre wire). Run up here, grab these weapons, go to profs room, take him out and wait right there for the SWAT.

While this may not end up being the fastest way to get through the level, it would be one of the most quiet, in that your "The SWAT team is blowing the hell out of me" factor is dropped significantly. (Note: You may want to press escape to skip the cutscenes, to give a bit of extra time to you.)

When you first enter the hospital and get past the orderly's jabbering, of course, run like the wind up the stairs and through the operating theatre; if you make good time, you may make it past that room and into the hallway beyond before the first cutscene takes place and the SWAT are called.

Instead of hanging a right into the good doctor's office, however, run straight forward and into the restroom; open that first aid kit and pull out the syringe, then run into the doctor's office afterward. Make sure, however, that you don't let him run.. instead, finish him with the syringe, then drag his body behind the desk. Steal his clothes.. and just stroll right out of the office and past the SWAT units. And if you've got the patience, they'll run around the level, opening the majority of the doors for you in their search for you. Best part is, they'll never notice the doctor's corpse.

Here is a strategy for completing the Setup level without fighting the SWATs or needing to rescue the CIA guy: First run round the level and get whatever weapons you want to take with you to the next level, i.e. uzi syringe, etc. Then kill Kovacs and take his clothes. Take the sanitarium key (if you want), then take the elevator down to the ground level. Whilst in the elevator click on the elevator button to bring up the menu. Now turn to face the exit whilst making sure that the menu remains on the screen. Then very quickly click the button to send the elevator up and run out of the elevator before the doors close. The hidden exit to the basement will now be exposed and you may proceed to the next level.
